Project HOME

The Hospital to Home (H2H) program seeks to link medical and housing resources to provide supportive housing to individuals with Opioid Use Disorder (OUD) directly following a hospital discharge. This program is made possible by the Estadt Lubert Collaborative between Penn and Project Home.

Penn has a dedicated number of beds allotted for referrals. If eligible and admitted to a residential site, patient can receive medical case management, peer support, collaboration between housing and medical partners, transportation to appointments, copay assistance, items of daily necessities, and more. 

All patients, regardless of engagement in this program, are able to access walk in resources at 3801 Filbert and 265 E Lehigh where Penn CAMP staff are available to provide navigation Monday-Friday 9-4pm.

Project HOME also supports workforce training and offers skills in various fields throughout the year, including computer-based website building, music production, ServSafe Food Handler certification, certified peer specialist training, and more. Find out here!
